All first-time clients will receive massage at the special discounted rate. But since we’ve not worked with you previously, we do ask that you complete two simple forms.

The first is an intake form which consists of questions about your general health history and any recent or past injuries you may have sustained. The second is a client policy that outlines the basic policies regarding massage therapy at New Horizons Massage.

You can access and print these forms, complete them at your leisure and bring them with you. Or, you can arrive at the office 10 to 15 minutes early and complete them prior to the start of your therapy session.
